Choose which service, Click Request to Book, then click “Schedule Appointment” Choose Date & Time click “Request to Book”

Fill out Client Booking Form then click “Request to book”. Please give me some time to review your request once I approve your appointment you will receive multiple emails the last one should include your invoice.

Click “View Invoice” Review info and then click the blue button “Pay Invoice”

Choose which payment method “Credit Card, Affirm or Afterpay” (once you click to proceed, you will be redirected to either site for payment or application.)
Please Note: *(InkySPA charges an extra 3% for all debit & credit card transactions, 5.99% + 30¢ per transaction fee for Affirm and extra 4-6% + 30¢ per transaction fee for Afterpay. *All transaction fees must be paid upfront & separately *subject upon approval

*If requesting tattoo appointment please attach reference example pictures and pictures of placement (arm, leg etc) take pic of body part & circle area.
* Your Budget for Tattoo (50% Half Down Non Refundable Deposit Required to Book)
* Your Availability - What date you can come for appointment? How soon are you looking to get started?

Fill Out Wavier Release Form
Step 2:
Ready to book your appointment? InkySPA requires all clients new and returning to fill out wavier release form each time you request a new appointment. To ensure we meet personal needs and to better assist you it is important for your safety and proper healing that you disclose any relevant medical history; however, this does not legally prevent you from undergoing a body art procedure or skin care treatment. Rest assured, all your information will be kept strictly confidential. Once you've filled out form please text "Wavier Form Completed" to 708-401-7599 and we will get back to you shortly once forms are reviewed.

Send Deposit to Lock in Appointment
Step 3:
InkySPA requires a half down 50% Non Refundable Deposit. ALL DEPOSITS ARE NON-REFUNDABLE & NON-TRANSFERRABLE!
